hi po mag request po ako ng vaccine card ko kasi nawala po sya nung july 19 2023 dahil nalaglag ang wallet ko hindi ko naman po matandaan yung exact date ng vaccine ko ng 1st dose and 2nd dose kaya hindi ako makapag request.ako po si maricel bautista.
