Hi! I would like to request some information and data regarding the following:

1) Field Health Service Information System (FHSIS) for year 2017 of RHU Angadanan, Manango Hospital and Lucas-Panguila Hospital in Alicia, Isabela;

2) Provincial data of Diabetes', Chronic Obstructive Pulmonary Diseases, Cardiovascular Diseases and Cerebrovascular Diseases incidence rate from 2014 to 2016

Our FHSIS Unit does not have disaggregated  FHSIS Reports for specific LGUs.
