Hi! I would like to request to correct the surname in my Covid-19 Vaccination card. The name listed is Fathmah Abdul Chua (my ex-husband’s surname) to my present name (Maiden name) Fathmah Monzon Abdul. I will be needing the correct card for my vaxcert. Kindly let me know how this issue can be resolved.

You asked for Change name for my vaccine card.

Response to Request

Please be informed that the encoding of
vaccination records is done by the concerned
vaccination center. The vaccination record may
have been incorrectly encoded in the Vaccination
Information Management System (VIMS) by the
vaccination center.
Kindly coordinate with the LGU where you
received your vaccination and request for
assistance to correct your vaccination record and
certificate.
