Dear Sir/Ma'am,

Good Day!

I would like to request a validation on my vaccination. I requested my vax certificate online in your website last July 19, 2023 but it shows that I have no record both of my 1st and 2nd dose.

I have my vax card with my 1st dose, 2nd dose and both of my 1st and 2nd dose of boosters. However when checking it on your website vaxcert.doh.gov.ph No record found in my 1st and 2nd dose. I badly needed it asap since I am planning to apply abroad this month.

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph  or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.
