Good day! I would like to ask for a copy of the data regarding online health facility statistical report being mandated by DOH via https://ohsrs.doh.gov.ph/ annually submitted by some hospitals in DOH Region 4-A from 2015-2022 as follows:
a. General Emilio Aguinaldo
Hospital
b. De La Salle University Medical
Center
c. Philippine Health Center
d. Divine Grace Medical Center

Based on the Online Health Facility Statistical Report System (ohsrs.doh.gov.ph) portal the General Emilio Aguinaldo Hospital did not submit their annual statistical reports on the requested period. Meanwhile, the Divine Grace Medical Center, submitted for the period of 2017-2022.
