Hi! I would like to request for a vaccination certificate. However, I lost my vaccination card last week. I am fully vaccinated but I don't have any proof other than my vaccination card. My vaccination data are as follows:
1st dose - 08/09/21 Brand: Sinovac
2nd dose - 09/06/21 Brand: Sinovac
Booster - 02/04/22 Brand: Sinovac
Venue: City Mall Goldenfield, Bacolod City
