Hi! I'm requesting to get a copy of my brothers Vaccination certificate as he lost his vaccination card.  His name is John Daniel M. Degano from Blk 1 Lot 17 Camella Townhomes 1 Putatan Muntinlupa City born January 7, 2006.  We are unable to retrieve it online as we don't know when exactly he got his 1st and 2nd doses of Vaccination.  Can you still provide us a certification saying that he is fully vaccinated as we needed it for his doctor's appointment tomorrow morning? Please email it to my email address meechdevera@gmail.com. with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
