Good day.

I'm writing this request as a follow-up to my previous one dated March 22, 2019 with request number #DOH-948636994421. 

On March 25, FOI officer endorsed my request to the DOH Disease Prevention and Control Bureau (DPCB). I asked for a follow-up, to which the FOI officer responded on April 17 that they have yet to receive a response from the DPCB. Since then, I have been asking for follow-ups with regards to the status of my request, and up to this date I haven't received any updates. 

I still need the data and I hope that this issue will be addressed soon.

While our aim is to provide information whenever possible, in this instance this Office does not have [some of]* the information you have requested. However, you may visit the National Health Facility Registry at http://nhfr.doh.gov.ph for the masterlist of all health facilities
