Hi!

Can you help me to change my birthday details on my vaccination card it was wrongly input, insteas of 20 they wrote is 19, it might having a problem on this.

You asked for Updated Birthday listed on my Vaccination Card.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. The vaccination record may have been incorrectly encoded in the Vaccination Information Management System (VIMS) by the vaccination center.

Kindly coordinate with the LGU where you received your vaccination and request for assistance to correct your vaccination record and certificate. Thank you.
