Good day! I would like to request an update regarding my covid claim/assistance status

Last September 8,2023 an email was sent to me by one of your department and it was indicated that my voucher is approved and still waiting for the availability of funds. Some of my colleagues already received their claim last 2023 and the period we were infected by covid virus and filed for compensation were almost close. I would like to kindly ask the status of mine. Thank you for your kind consideration and hoping for a detailed response.

1. Name of the Hospital or Health Facility  you are currently employed?
- I was working under Jose B. Lingad Memorial Regional Hospital located in City of San Fernando, Pampanga during that time I submitted my requirements for my covid claims/covid compensation assistancefor the frontliners who were infected with Covid virus and now I'm currently not working under them anymore.

-Nursing Service --- NURSE I-Job order

3.  Did the facility submit the documentary requirements for Health Emergency Allowance (HEA). If so, which DOH office did they submit the requirements to?
- I would like to clarify I'm not following my HEA allowance,it was already given as a whole. My concern is regarding my covid claims/covid compensation assistance for the frontliners. Jose B. Lingad Memorial Regional Hospital submitted my requirements to Department of Health CLCHD III (Region 3). Attached to this was the last email of the Department of Health CLCHD III it was dated last September 9,2023. Thus, I want to follow up the status of the voucher specified in this email.
