Hi i would like to request the information of my lost vaccinatiom card to be able to register on vaxcert because i forgot the date of my second dose vaccination. Thank you .

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
