Hi! My name is Angelica Tenorio Hernandez and I would like to humbly request to change my Vaccine Certificate Surname. I used my Father's surname (Tenorio)  when i am single and my application vaccine certificate, and now i am married I used my husband surname thats Why I humbly request to change it to ANGELICA TENORIO HERNANDEZ

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. 

If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to update the details in your vaccination certificate. 

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
