Hi! I would like to request another vaccine card because mine got lost. I dont have any pictures of my vaccine card however I remember when and where I got vaccinated. I got my first dose at August 10, 2022 in Buhangin Gym. I tried searching for my record in VaxCertPh, it says that I dont have any record. I hope my request gets fulfilled for I will be using it for our swimming class tomorrow.
