Dear Sir/Madam,

I am writing to request a copy of my COVID-19 vaccination card as I have unfortunately lost the original copy. I received my vaccination at sagay city . It would greatly help me to have a record of my vaccination as it may be required for future travels or events.

I hope you can provide me with the necessary details such as the vaccine brand, lot number, and date of administration. If there are any fees or documents necessary to obtain a copy, please let me know.

with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
