Good Evening. I just want to ask if can I request for a new copy of vaccination card? Or replacement or where can I request a new one? I needed it to the company that I applied to. I lost my vaccination card together with my other valid ID’s when I got robbed in the jeepney. My full name is Mary Jane Basas Malibago, birthdate was December 16, 1997, my 1st and 2nd dose brand was Pfizer. I got my 1st shot on January 15, 2022 and the 2nd was on February 5, 2022. Lot No is PCA0028 and the vaccinator name is RN Charina Gene P. Dayan. I received it at Silvina Covered Court Novaliches Quezon City.

About the copy of vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
