I would like to raise a concern regarding my vaccination record. I lost my vaccination card, I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination. 

I was vaccinated in Angono, Rizal in SM Angono. My first dose was on December 2, 2021 and the second dose was at January 14, 2022. As I have said above I lost my vaccination card and wasn't able to take photo of it as a proof of it. Please send me a message thru my email Atreeguevarra16@gmail.com for more information needed regarding with my concern.

Response to Request

Your FOI request is approved. 
About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
