     This is ANNA Linda AQUINO, I have loss my vaccination card. I may request for duplicate or any prof I can get to show when I applying for Job. The details when I get my vaccination are Name of vaccine: Zinovac, the 1St. dose in July 20-22, 2021 and the 2nd dose in August 19, 2021. Held on Pasay - mall or facility name are HDN/ CLI CBCR 07970. Coronavac Covid -19 vaccine (vero cell). Th Please can you help me on my request. Response to Request

About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.


Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to the Central Appeals and Review Committee, at foi@doh.gov.ph.
