Hello. Im asking for assistance about my vaccination record.I requested a vaccination certificate online however my booster dose doesn't show on the record. We will urgently need it to apply for a yellow card as a requirement for work abroad.Looking forward to a positive response from you.

You asked for Booster Dose info incomplete on Vaccination certificare online.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
