I am Orly Dave Judith , 24 yrs old, residing at #9 General Lizardo St. Barangay Dioquino Zobel Project 4 Quezon City. I would like to request update of  my 2nd  dose details for my vax certication ,I need to have a copy of my vax certification for my ongoing application abroad in Taiwan. 

My second dose was in Mandaluyong City dated September 21,2021, with a LOT # A1059 .
