Good day!i would like to request my vaccination record
Hello there! I would like to raise a concern about my vaccination record. My mom is furious of how I lost my vaccination card. I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination. 
I was vaccinated in Robinson Place,Imus Cavite. My first dose was on December 03,2021 and i take the second dose in Buhay na Tubig vaccination center but i cant remember the date .As I have said i lost my vaccination card and also wasn't able to take the photo of it.
