Good day! I am humbly requesting for the following information to be used for Spatio-temporal Analysis:

1. Weekly Report of Cardiovascular disease cases in the Philippines per case/person 
   a. Demographic data of the patients (e.g. age, sex, location)
   b. Risk factors (e.g. diet, cholesterol intake)
2. Prevalence and Incidence data
3. Mortality data of the patients

Data format: CSV, Excel, or GeoJSON

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. 

We only have data on the FHSIS reports on the Diseases of the Heart posted at the DOH website https://doh.gov.ph/publications/serials
