Please be informed that we could only endorse this request to DOH Hospitals only.  We do not have this data in private and LGU Hospitals
KIndly check on the DOH website at https://doh.gov.ph/doh-hospitals-directory. 

Please let us know which of these DOH Hospitals we are going to refer this request..
