Greetings, ma'am/sir! 

I am a third year medicine student and our group wants to assess the PHILPEN protocol that was advocated by Loguilo, ALubjid, Cagayan de Oro City. We want to know if you can provide us with the following data:

(1) Number of patients diagnosed with hypertension and diabetes before and after the implementation of the protocol in Loguilo, ALubjid, Cagayan de Oro City
(2) Copy of the PhilPEN Ordinance of Loguilo, ALubjid 

(3) Monitoring and evaluation (M&E) reports showing that the PhilPEN ordinance is implemented well or not.

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. 

We regret to inform you that we do not have custody of the data requested and you directly coordinate with the Municipality of ALubjid, Cagayan de Oro City since the data request is specific to Barangay Loguilo only
