hi, i would like to request a vaccine cert for travel abroad but i lost my vaccine card and forgot the date i had recoieve my vaccine.

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
