My Cousin name: Cristal Ann Austria Ono
Birthdate: Sept 25 1993

We Just want to confirm only if she was born at QMMC. Because the QMMC right now have no record that year 1993, They already disposed the all the medical records that year.

While our aim is to provide information whenever possible, in this instance this Office does not have  the information you have requested. 

We regret to inform you that QMMC no longer has custody of the requested information and we request you coordinate with the Philippine Statistics Authority (PSA) or the Quezon City Records for this concern
