God Day,
I was hired as job order during Covid 19 nurse in the province of Capiz,till now i did not recieve my Health Emergency Allowance from July 2021 to March 2022.Hope you can respond on my request.

1. Name of the Hospital or Health Facility  you are currently employed?
2. Job description/ Nature of work  (what is your Department/Section/Unit?)
3.  Did the facility submit the documentary requirements for Health Emergency Allowance (HEA). If so, which DOH office did they submit the requirements to?

Good Day,
We were hired as a job order from Provincial Department of Health in the Province of Capiz starting from July 2021 and ends at March 2022,and we are deployed as Covid triage Nurse in Roxas Memorial Provincial Hospital in the City of Roxas.As we all know our PDOH transmitted all the data to Regional office region 6 which is located in Iloilo City.Attached is our data that supports our claim.
