Hi! I would like to request a document regarding: 

1. My daughter's immunization records from when she was a baby. Her name is Myesha Claire Galan. Born in November 25  2015. The center she was vaccined to is located in Brgy. 28 Caloocan City. 

This will be used for visa application and her scholl enrollmemt in Australia.

You asked for Immunization Records of my daughter.

Response to your request

While our aim is to provide information whenever possible, in this instance, we regret to inform you that we do not have custody of barangay immunization record. We ask that you coordinate with the Barangay Health Center  where your child received the immunization for this request
