To whom it may concern: 

This is to bring to your attention that I have been trying to update my vax cert 
from 
MARTINEZ, CECILE JOY UBANTE 
to 
GIMUTAO, CECILE JOY MARTINEZ 

However, I still got no response from the system. I really need this vaxcert for our travel to Japan on Aug/9/2023. Please help me as soon as possible i'm running out of time, I already submitted the copy of my vaccination card and my passport in your system,

You asked for VaxCert Update Request.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph  or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.
