Hello!
I would like to request correction of my birth year in my vaccination record and certificate on vaxcertph. 
The website / vaxcert record for my birth year is set to 1994, but the correct year is 1984. I really need to change my birth year asap because I have a scheduled flight.

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is incorrect, kindly coordinate with the LGU at General Trias where you received your vaccination and request for assistance to correct the details of your vaccination certificate.
