Hi. I would like to request information regarding the number/cases and mortality rate of of heart diseases in 2019-2022 in the following Municipalities/ Cities in Central Luzon particularly:

1. Bamban, Tarlac
2. Capas, Tarlac

While our aim is to provide information whenever possible, in this instance this Office does not have  the information you have requested. 

 We regret to inform you that we do not have related records of mortality cases/rate particularly Bamban, Tarlac and Capas, Tarlac available  at the RESU/FHSIS Office in the DOH Regional Office 3 Central Luzon

May we suggest that you ask for the data needed with the Provincial Health Office of Tarlac with these following emails.
