Hi, I been for the applying for the Issuance of Vaccination Certificate but the is one missing information:
 1st Dose: Sinovac  held on 30 June 2021 (Manila) -  with record
 2nd Dose: Sinovac held on 28 July 2021 (Manila) - with record
 Booster-1: Pfizer held on 04 January 2022 (Imus, Cavite) - Missing (Health Facility Name: Maharlika Site) Batch/Lot No.: PCA0024
  
 Booster-2: Pfizer held on 21 September 2022 (Quezon City) - with record
