I would like to raise a concern regarding my vaccination card. I lost my vaccination card last August 2023 when we move into a new house.

I was vaccinated in via temporary vaccination site at Dasmarinas, Cavite. My first dose was on November 24, 2021 and the second dose was at December 22, 2021. As I have said above I lost my vaccination card and wasn't able to take photo of it, but I do have pic of proof.

Response to Request

About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.


Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to the Central Appeals and Review Committee, at foi@doh.gov.ph.
