Hi! I would like to request to remove my middle name and correct the surname in my Covid-19 Vaccination card. The name listed is Joyce Ann Lavilla Alvarez since that’s the name that I was using before I got my Philippine passport. But my present name is the name on my birth certificate which is Joyce Ann Lavilla. All my documents and Government Id’s are already updated. I will be needing the correct card for my medical requirements at Saint Lukes Medical Center on December 22, 2023, this is for my travel purposes (US Immigrant). Kindly let me know how this issue can be resolved.
