From JOLINA KONG AQUINO to JOLINA AQUINO SANTOS. 

Purpose: To update all my IDs and records for travel abroad documents.

Please be informed that the encoding of vaccination records is done by the concerned vaccination center. kindly coordinate with the LGU where you received your vaccination and request for assistance to update your vaccination certificate. 

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
