I would like to raise a concern regarding my vaccination record. I lost my vaccination card last week, July 2023. I'm fully vaccinated but I don't have other copy of my vaccination card. I was vaccinated in Kawit, Cavite for my first and second dose, as well as my first booster shot. My first dose was on October 6, 2021 and the second dose was on October 27, 2021. My first booster was on February 3, 2022. I was able to take a picture of vaccine card, but I was not able to take a picture or my second booster card.

About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
