I want to express a worry about my vaccination record. Despite the fact that I am properly vaccinated and have lost my vaccination card, I do not possess a backup. I had two doses of the vaccine in De La Salle-Dasmarinas Gym in City of Dasmarinas, Cavite: one on December 23, 2021, and the other on January 12, 2022. My vaccination card was misplaced, as I said above, and I was unable to snap a photo of it. I want to know if I can get a copy since I need as a requirement for my medical examination in August 30,2023.

About the lost vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
