Hi! I would like to have my vaccine certificate because it lost with my wallet someone got stole it from inside of our house.
I hope I can immediately have a copy , thankyou!

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
