Response: Province of Cavite for the year 2018-2019-2020
2. Request submissions of Annex E of DOH AO 2012-0012 from Cavite licensed hospitals.
- What year are we requesting?
Response: for 2018-2019-2020
- Specific Hospitals?
Response: No specific hospital for Annex E of DOH AO 2012-0012, to have a larger population size in the scope of the study.
