Hi. I recently tried to retrieve my vaccination record through https://vaxcert.doh.gov.ph, but I received a notification indicating that no records were found under my name.

The following are my vaccination details:
Full name: Jumeida M. Mahamod
1st dose - 07/30/2021
2nd dose - 08/27/2021
City/Municipality: Kalawit, Zamboanga Del Norte

I would greatly appreciate it if you could respond to my request ASAP as I will be traveling this Saturday , September 16, 2023. Thank you for your kind and immediate assistance.

You asked for Vaccine certificate.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.
