Good day! I would like to request for my immunization record for myself as some of the dates for my vaccines are not recorded like the varicella dose number 1. My vaccines were mostly done in Sta. Rosa Polyclinic in Laguna. I assume my varicella was given to me there too. I was born in 1993. Please help me locate my records. Thank you

You asked for Incomplete Childhood Immunization record.

Response to your request

While our aim is to provide information whenever possible, we regret to inform you that DOH does not have custody of immunization records. We ask that you directly coordinate with the health center where you  received your vaccination or you may coordinate with the Municipal Health Office in Laguna LGU for further assistance.

Your right to request a review
