Hello Sir/Madam,

I hope this message finds you well. I am writing to request a copy of my vaccination card which I misplaced on my cellphone that was stolen. I require the documents as I am in the process of applying for a job, and it is one of the requirements necessary to proceed to my final interview. 

I apologize for the inconvenience, but I forgot the dates when I received my vaccinations. I received both my first and second doses of the Sinovac vaccine in Tagaytay City, and I have also received my booster shot. Could you kindly provide me with the dates and a copy of the document.

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
