To whom it may concern ,

   I would like to change my Birthdate on my vaccine card I’ve written the wrong birthdate when i was signing up for my 1st dose Please change it from OCTOBER 19,1998 To OCTOBER 19,1999 I might run into some problems in the future with any of my government documents

Your FOI request is approved. 

Please be informed that the encoding of vaccination records is done by the concerned vaccination center.

Kindly coordinate with the LGU at Lapu-lapu city, Cebu where you received your vaccination and request for assistance to correct your vaccination certificate. 

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435.
