Kindly coordinate directly with the National Patient Navigation and Referral Center (NPNRC) at Telephone number (02)8651-7800 local 2431-2345 or hotline number 1555 local 2 for immediate assistance.
Please prepare your reference number generated thru Vaxcert.doh.gov.ph for verification purposes and ease of transaction.

Your FOI request is approved.
About the copy of your vaccination card, kindly coordinate with the LGU where you received your vaccination as their office shall verify your record prior to the issuance of a new vaccination card. Thank you.
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
