To Whom it May Concern!

I would like to request if possible that i can have a copy of my Immunization record year 1984 (year of my birth) to 1985. The said record is necessary for the process of my application as a requirement from my employer overseas for a successful job employment. I do hope you can provide an assistance for this.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have the information you have requested. 

We wish to inform you that we do not have custody of immunization records. We suggest that you coordinate with the health center where you received your immunization for this request
