Hello,

I hope this message finds you well. I recently realized that I've misplaced my vaccination card, which I received from your organization. I understand the importance of having an accurate record of my immunizations, and I'm concerned about the loss.

Is there a process in place to request a replacement vaccination card or obtain a copy of my immunization records from your facility? I would greatly appreciate your guidance on how to proceed in this situation.

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
