Would like to raise a concern regarding my booster record. I lost my vaccination card last October. I'm fully vaccinated but when I checked my record through https://vaxcert.doh.gov.ph/, it says that they have no record of my vaccination. 

I was vaccinated in City of San Jose Del Monte Bulacan. My first dose was on August 3, 2021 (Janssen)  and my Booster (Pfizer) was at August 6, 2022. As I have said above I lost my vaccination (Booster) card and wasn't able to take photo.

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.
