Hi! I would like to follow up my Vax Cert Request with: 

1. Certificate ID: 936617767

To whom it may concern: 

This is to bring to your attention that I have been trying to access my vaccine certificate numerous times since  December 2021. 

However, nobody calls or even email me and I really need this vaxcert for my visa application to Japan. Please help me as soon as possible because I am running out of time, Kindly tell me where can I submit the copy of my vaccination card. Here is my email address (wendelldeasis@gmail.com )

Response to Request

Your FOI request is approved. 
Please be informed that the encoding of vaccination records is done by the concerned vaccination center. If your vaccination record is unavailable, either the record may not have been uploaded yet in the Vaccination Information Management System (VIMS) by the vaccination center.
If you want, you may kindly coordinate with the LGU where you received your vaccination and request for assistance to generate your vaccination certificate. 
In the meantime that we are experiencing Technical Issues on our VaxCertPH portal, kindly refer to the attached DOH Memorandum Circular for other options.

For further assistance, you may call the Vax Cert Customer service at (02) 8876-1488 and vaxcertsupport@doh.gov.ph so you can provide us the reference number from the VaxCert website or the National Patient Navigation and Referral Center (NPNRC) at (02) 8651-7800 local 2431-2435. You may also review the FAQ section of the eFOI site: https://foi.gov.ph/help.
