Your FOI request is approved. 

Visit the DOH National Health Facility Registry (NHFR) website at https://nhfr.doh.gov.ph/VActivefacilitiesList for the master list of health facilities in the Philippines, including the Drug Abuse Treatment and Rehabilitation Centers (DATRC)

Please note that we are only able to provide some of the information you have requested for as we do not have custody of data for Physical Rehabilitation Clinic. Kindly coordinate with the Department of Social Welfare and Development (DSWD) or the LGU for this request
