Good Day FOI,

I would like to ask request on DOH to update my vaccine record since my 2nd dose shot does not yet appear. I will be using the details to travel abroad. 

Here are my details:
Origin: davao city

1st dose: Sinovac 08/10/21 city of davao
2nd dose: Sinovac 09/07/21 city of davao
