I lost my vaccination card is there anyway to get a new one??i worry that maybe its still needed in some places i got vaccinated November 7 2021 and December 27 2021 i am fully vaccinated i got vaccinated at montessori sagrada at tangos baliwag bulacan

About the lost vaccination card, kindly coordinate
with the LGU where you received your
vaccination as their office shall verify your record
prior to the issuance of a new vaccination card.
